Report of marriage

Hi everyone hope you are all ok.I just want to ask regarding report of marriage form,i am married,in the report of marriage form last name what should i put my maiden name or married name and in the civil status the option are single,divorced and widowed i dont know what to put.thanks

Ah! There is a thread on that subject. If I recall correctly, you have to complete the Report of Marriage, then renew the Passport.

Can i do it on the same day?

Years ago my ex wife (who is also Filipina) and I married in Hong Kong and she got it done by the Consulate there on the same day. So I expect it can be done, but I would definitely advise going to the Embassy in person and taking all the forms and the documents and the copies that will be needed.
