another snag when trying to pay the NHS surcharge it keeps being declined have spoke to the bank no problem have tried different browser 5 times enabled cookies its refused start new application 5 times declined again what a rubbish set up,lets me pay online for phone call to them and email when you phone its 1.37 pounds a minute and 6 pounds nearly to email, will have to wait until back to U/K to sort it out another delay frustrating is an understatement, they did send an email back after my inquiry no help at all, absolute disgrace of a system you can't do official compliant because it won't take payment any ideas from anyone had a similar time?

What a shame that is. I don't know what to tell you coz you seem to have tried everything. Hopefully when you are back in the UK you can pay it no problem.

And by the way she brings her passport and a copy of her application form with her to the VFS.

The VAF4A form is basically the online application so you don’t have to print another one and fill it as it will be on the online application. The Appendix 2 will definitely have to be filled in as that is for meeting the financial requirement. SU07 or the sponsor form you are saying is not a requirement but a lot of people submitted it, including myself. Wouldn’t hurt to submit it.

    Hi Everyone.
I hope you can help me. I have a question regarding a change of name. I'm planning to change my name to my spouse surname. If I submit some of my payslips and bank statements that under my maiden name are they going to accept it or not? Thank you so much.

Yes they will accept the payslips in different names.
Your marriage certificate will be available to them and your payslips will have your national insurance number on them.

Sorry I'm asking that question as my tbs bank didn't accept our marriage certificate as I want to change my name. They said they can't accept it as it looks like a copy. But it was from PSA(former NSO).

Haha funny you should say that ours was refused by the Philippines embassy in London as it was not on the right coloured paper it was yellow on the front and white on the back but with the correct stamps on.
The correct paper should be yellow on the front and pale yellow as if the colour had soaked through
